Job Description :
The Speech-Language Pathologist (SLP) will provide comprehensive therapy services to patients within a skilled nursing / rehabilitation environment. The role involves assessing communication and swallowing disorders, creating individualized treatment plans, and delivering high-quality, patient-centered therapy.
Responsibilities :
- Conduct evaluations of speech, language, cognitive-communication, and swallowing disorders.
- Develop and implement individualized treatment plans based on patient needs and clinical best practices.
- Deliver therapy sessions focused on functional communication, swallowing safety, and cognitive rehabilitation.
- Maintain accurate, timely, and compliant documentation of assessments, progress notes, and discharge summaries.
- Collaborate with interdisciplinary team members including PT, OT, nursing, and case management.
- Educate patients, caregivers, and staff on therapy goals and strategies to support functional improvement.
- Participate in care plan meetings and contribute to overall patient management discussions.
- Follow all regulatory, professional, and ethical standards.
Requirements :
Active and unrestricted SLP license in the state of assignment.Master's degree in Speech-Language Pathology.Certificate of Clinical Competence (CCC-SLP) preferred.Previous SNF or rehab experience strongly preferred.Strong communication, documentation, and interpersonal skills.
