Role Overview :
The LPN will provide in-home nursing care to patients under the supervision of a Registered Nurse. You will be part of an interdisciplinary team dedicated to ensuring comfort, quality of life, and dignity for every patient and their family.
Your Key Responsibilities Will Include :
Administering medications and treatments following hospice protocols and nursing standardsPerforming essential clinical tasks within the scope of LPN practiceMonitoring changes in patient condition and notifying the RN as neededContributing to the development and revision of care plans with measurable outcomesMaintaining complete and timely documentation of all care activitiesOffering emotional support to patients and loved onesParticipating in care coordination meetings and collaborating with the broader care teamEncouraging teamwork by supporting other staff as neededIdeal Candidate Qualities :
Graduate of an accredited LPN or LVN programHolds an active LPN license in the State of New MexicoCPR certification requiredPossesses a valid drivers license and auto insuranceMinimum of 1 year of nursing experience (hospice experience is a plus)Strong interpersonal and organizational skillsA compassionate, professional approach to patient careComfortable with basic math and medication administration proceduresAdditional Expectations :
