• Compensation : - $270,000 to $315,000 based on experience

• Additional Info : At Urban Health Plan (UHP) our mission is to continuously improve the health of communities and the quality of life of the people we serve by providing affordable, comprehensive, quality, primary, and specialty health care and by assuring the performance and advancement of innovative best practices.DETAILS : - Provides bio-psychosocial assessments and clinical diagnostic assessments.-Responsible for medication management and providing continuity of care-Patient volume consists of 18-20ppd, with a mix of new patient intakes and follow-ups : New patient intake appointments range from 45 minutes-1 hour, follow-ups are 15-20min. -Opportunities for a hybrid-remote schedule available once continuity of care is established-Psych NP and psychotherapist staff on site to provide support- Participates in clinical team meetings.QUALIFICATIONS : - NY State License and DEA REQUIRED- Experience in community healthcare and / or bilingual highly desireable
