Compensation: $115,000 – $125,000 annually

Position Summary

We are adding an LCSW to our integrated care team to provide brief therapy and support care coordination for adult patients. This role offers a strong balance of clinical work, collaboration, and practical patient impact.

Key Responsibilities

• Conduct assessments and provide evidence-based brief interventions
• Screen for trauma and substance use concerns and coordinate next steps
• Partner with primary care providers on treatment planning and follow-up
• Provide referrals to specialty care and community resources
• Complete documentation in the EMR within required timeframes
• Participate in case reviews and interdisciplinary meetings

Requirements

• MSW from an accredited program
• Active CA LCSW license (ASW accepted; supervision provided)
• Minimum 1 year of clinical experience
• Familiarity with DSM-5 and evidence-based practice
• Basic computer skills and EMR proficiency

Preferred Qualifications

• Community health experience
• Trauma-informed care training
• Spanish fluency is a plus
