Social Worker (LCSW) – Chronic Disease & Dialysis Services
Position Summary
The Social Worker provides professional psychosocial assessment, counseling, care coordination, and supportive services to patients with chronic diseases — including Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and related conditions — receiving in-center or home dialysis. The role focuses on improving patients’ quality of life, treatment adherence, and psychosocial well-being in collaboration with the interdisciplinary care team.
Key Responsibilities
Conduct comprehensive psychosocial assessments for patients with chronic illness or undergoing dialysis to identify social, emotional, financial, and environmental needs.
Develop and implement individualized care plans in collaboration with physicians, APP, nurses, dietitians, and care coordinators.
Provide emotional support and counseling to help patients and families adjust to chronic illness, lifestyle changes, and treatment demands.
Facilitate crisis intervention , grief counseling, and coping strategies for patients and caregivers.
Assist patients with accessing community resources , such as transportation, financial aid, medication assistance, mental health services, and housing support.
Educate patients and families on treatment compliance , dialysis expectations, and advance care planning.
Advocate for patient needs to ensure appropriate coordination between healthcare providers, dialysis centers, and community agencies.
Maintain knowledge of current ESRD regulations , CMS Conditions for Coverage , and social work best practices in chronic disease management.
Qualifications
Master’s degree in Social Work (MSW) from an accredited program.
Current state licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW). Texas and Florida Licensure.
Minimum 2 years of experience in medical, renal, or chronic disease settings (preferred).
Strong knowledge of dialysis treatment modalities , chronic illness coping mechanisms, and community resource navigation.
Excellent communication, documentation, and organizational skills.
Ability to work collaboratively in an interdisciplinary healthcare environment.
Work Environment
Remote and Telehealth Network
Interdisciplinary collaboration with physicians, nurse practitioners, nurses, dietitians, and case managers.
