The Antibiotic Stewardship Clinical Pharmacist will serve as a unit-based pharmacist, verifying orders and providing specialized clinical services in antibiotic stewardship and infectious disease. This role will be a critical drug information resource for nurses, physicians, patients, and families.
Responsibilities:\nProvide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
\nRoutine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions.
\nDeliver medication therapy management with a focus on antibiotic stewardship and infectious disease protocols.
\nEnsure pharmaceutical care programs are appropriately integrated throughout the institution.
\nAssist in development and implementation of facility-specific protocols, guidelines, and reference documents related to antimicrobial therapy management.
\nConduct Medication Use Evaluations (MUEs) with a focus on antibiotics/ID, and recommend necessary changes to pharmacotherapy practice.
\nServe as a resource in antibiotic stewardship initiatives, ensuring best practices and compliance with institutional standards.
Qualifications:\nEducation
\nRequired: PharmD or BS in Pharmacy
\nExperience
\nRequired: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
\nStrongly Preferred: Infectious Disease training and/or experience in antibiotic stewardship
\nCertifications
\nRequired: Current Pharmacy license in Massachusetts
\nPreferred: Board Certification in Pharmacotherapy (BCPS) or actively working toward certification
