Position Highlights :

• Position : Rehabilitation Clinical Liaison
• Location : Northwest Community Hospital, Arlington Heights
• Full Time / Part Time : Full time
• Hours : Monday-Friday, 8 : 30am - 5pm; Rotating weekends- 1 weekend per month (May be additional coverage needs on a month with 5 weekends)
• Required Travel : no
• Holidays : yes; 1 winter and 1 summer holiday

The Clinical Liaison- Rehabilitation serves as the representative for the Acute Inpatient Rehabilitation Facilities (IRF's) within the Endeavor System. They educate the community, build referral networks, and cultivate physician and case manager relationships both internally (within Endeavor Facilities) and externally. The Clinical Liaison- Rehabilitation is responsible for completing timely and accurate clinical assessment and admission of patients including nursing, medical, therapy, and financial pre-admission screening of patients referred to the Acute Inpatient Rehabilitation programs. The Clinical Liaison assesses patients' needs, reviews medical records, and communicates with care teams to coordinate safe transfer of patients to the Inpatient Rehabilitation Facilities within Endeavor.
