A well-established skilled nursing facility is seeking a compassionate Social Service Director in the Streamwood Area.

This leadership role is ideal for someone who thrives on making a difference in the lives of seniors and their families, while also guiding a team of dedicated staff.

Key Responsibilities:

• Oversee and coordinate social services for both short-term rehab and long-term care residents
• Conduct psychosocial assessments and care planning
• Serve as a liaison between residents, families, and interdisciplinary teams
• Ensure compliance with federal, state, and facility policies
• Support residents' emotional, mental, and social well-being through advocacy and guidance

Qualifications:

• Bachelor’s degree in Social Work or related field (LSW preferred)
• Previous experience in a skilled nursing or healthcare setting
• Strong knowledge of state and federal regulations (MDS, discharge planning, etc.)
• Excellent communication and organizational skills
• Leadership or supervisory experience preferred
