Assessing and monitoring patients to ensure their health and well-being. Providing direct care such as administering medications, wound care, and assisting with daily activities. Collaborating with a multidisciplinary team to develop and implement individualized care plans. Educating patients and their families about the rehabilitation process and necessary care. Maintaining accurate patient medical records and following policies and procedures for quality patient care. Working under the supervision of a registered nurse, advanced practice nurse, physicians.
