Position Summary :

The Physical Therapist provides a broad range of physical therapy services including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment consistent with the position qualifications, professional practices, and ethical standards.

Demonstrates accountability for and contributes to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary environment.

Essential Duties / Functions :

  • Conducts initial assessments to determine medical history, symptoms and needs.
  • Contributes to interdisciplinary care plans.
  • Provides appropriate therapeutic interventions.
  • Evaluates patient response to treatment and adapts treatment plan as necessary.
  • Documents the course of patient care including progress made and continuing need for treatment.
  • Provides patient and family education on tasks and resources that will assist with continued functional independence.
  • Communicates with patients, caregivers, and their families regarding patients’ progress.
  • Contributes to the delivery of cost-effective care.
  • Reports and complaints / grievances made by the residents to the Therapy Program Director. Reports incidents of suspected incidents of resident abuse to the Therapy Program Director and / or the Executive Director immediately.
  • Additional job duties as assigned.

    • Education, Training, and Experience :

  • Must possess a current Michigan license as a Physical Therapist.
  • Two years of PT experience in the long-term care setting is preferred.

    • Specific skills, knowledge, and abilities :

  • Knowledge of current federal and state laws and regulations that apply to the practice of Physical Therapy in a long-term care setting.
  • Has strong organizational and time management skills and the ability to prioritize

    • responsibilities.

  • Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Basic computer knowledge is required.
