This part-time nurse practitioner (NP) / physician assistant (PA) role will include patient care rounding in post-acute care in one skilled nursing facility located in Eugene, Oregon.
- Job Duties :
- You will be a part of a collaborative physician-NP / PA team environment providing primary care in a post-acute setting
- Care includes but is not limited to admissions, progress follow up, acute follow up, and discharge
- You will also work with the facility to transfer patients to the level and site of care most appropriate for their medical needs upon discharge including returning to home, independent living centers, assisted living, and / or memory care
- Our clinicians work closely with hospice, palliative care, and home care agencies to ensure a safe transition home
- Our clinicians are also responsible for timely documentation and billing that meets Medicare guidelines through TeamHealth's EMR (gEHRimed)
- Rounding takes place 1 day a week (Monday through Friday) with the flexibility to set your rounding day and daily schedule
