Your Key Responsibilities :

  • Conduct thorough initial and ongoing assessments to monitor patient progress.
  • Evaluate changes in patients' conditions and adjust treatment plans accordingly.
  • Document all treatment and skilled interventions accurately.
  • Educate patients, families, and caregivers to enhance recovery and promote established goals.

    • Essential Qualifications :

  • Valid Physical Therapist license or certification active in Arizona.
  • A minimum of one year of experience as a Physical Therapist.
  • Experience in a home care setting is highly preferred.
  • CPR Certification (demonstration course required).
  • Valid Arizona Fingerprint Clearance card.
  • No COVID vaccination required.
