About the Role:

The Home Health Field Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care to patients within their homes across the United States. This position focuses on providing personalized, compassionate care tailored to each patient's unique health needs, promoting recovery, comfort, and independence. The LPN will collaborate closely with healthcare teams, patients, and families to implement care plans, monitor patient progress, and adjust treatments as necessary. This role requires strong clinical skills combined with excellent communication and organizational abilities to manage diverse patient caseloads effectively. Ultimately, the Home Health Field LPN ensures that patients receive safe, effective, and respectful care in the comfort of their own homes, improving their overall health outcomes and quality of life.

• Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.

• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.

• Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.

• Assure the continuity of care through delivery of quality patient care.

• Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.

• Monitor assigned cases to ensure compliance with requirements of third party payor.

• Promote Agency philosophy and administrative policies to ensure quality of care.

Requirements

• Current LVN License in CO

• Valid Driver’s License

• Valid Auto Insurance

• CPR Certification

• HomeCare HomeBase experience preferred
