Description

Chapters Living, a comprehensive skilled nursing, independent living, assisted living, and memory care community, is seeking an experienced and compassionate social worker. The ideal candidate will oversee the social services department, ensuring that residents' psychosocial needs are met in compliance with federal and state regulations.

• Plan, develop, organize, and implement the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations.
• Conduct comprehensive psychosocial assessments for residents upon admission, quarterly, and as needed, to identify and address psychosocial, mental, and emotional needs.
• Develop and implement individualized care plans based on assessment findings, collaborating with interdisciplinary teams to ensure a holistic approach to resident care.
• Provide crisis intervention and support to residents and their families, addressing issues such as adjustment to placement, grief, and loss.
• Facilitate discharge planning, coordinating with residents, families, and external agencies to ensure a smooth transition to other care settings or home.
• Maintain accurate and timely documentation in compliance with federal and state regulations, including progress notes, care plans, and assessments.
• Supervise and provide guidance to social services staff, including hiring, training, performance evaluations, and professional development.
• Stay informed about changes in federal and state regulations affecting social services and ensure departmental compliance.
• Participate in interdisciplinary team meetings, quality assurance programs, and other committees as required.
