Key Responsibilities of the Travel Case Manager:

• Coordinate and manage patient care across the continuum to ensure quality and cost-effective outcomes
• Develop, implement, and evaluate individualized care plans in collaboration with interdisciplinary teams
• Facilitate discharge planning, ensuring appropriate post-acute services and follow-up care
• Assess patients' medical, psychosocial, and financial needs to optimize outcomes
• Monitor resource utilization and identify opportunities for process improvement
• Educate patients and families on treatment options, discharge instructions, and community resources
• Ensure compliance with hospital policies, CMS regulations, and accreditation standards

Requirements of the Travel Case Manager:

• Active RN license (Illinois or compact state) required
• BLS certification required.
• Minimum of 2 years of recent hospital-based Case Management experience
• Strong understanding of utilization review, discharge planning, and healthcare regulations
• Excellent communication, critical thinking, and organizational skills
