Community Access Staff

JOB SUMMARY:

In a group based setting, responsible for the implementation of a comprehensive therapeutic adult day program for adults with disabilities. Plans and conducts daily activities to facilitate group learning experiences and assists in the development of living skills consistent with the participant's objectives identified in the Individual Support Plan (ISP) or Plan of Care (POC) and within the program design.

• Provides direct assistance in self-help, socialization, and adaptive skills training, retention, and improvement to individual participants and group participants.
• Provides direct assistance in teaching such concepts as rule compliance, attendance, task completion, problem solving endurance, work speed, work accuracy, increased attention span, motor skills, and safety.
• Provides direct assistance in training, retraining, or improving the access to and use of community resources by individual or groups of individuals.
• Provides individual specific assistance such as personal care, and self administration of medication, for individuals enrolled in program services. (Examples: feeding, toileting, lifting and transferring)
• Implements behavioral support plans of individuals to reduce inappropriate and maladaptive behaviors and assist with acquiring appropriate adaptive skills and behaviors.
• Provides active support and direct assistance in participations in community social, recreational and leisure activities.
• Maintain work area – cleaning tables, chairs, and supplies daily/weekly as scheduled. Assures supplies are kept organized and in working order. Return supplies to designated area daily.
• Provides transportation as needed for individuals to community access activities.
• Completes all required documentation such as (progress notes, goal tracking sheets, HRST tracking sheets, seizure tracking, positing tracking, communication logs, activity calendars, production tracking, etc.) Assure all paperwork is submitted on time.
• Ensure that the environment, pace of activities, and type of activities are ISP (Individual Service Plan) directed. Physically participate with the individuals in the program in all activities.
• Assist in ensuring that all support services as well as site remains in compliance with all internal and external policies. Compiles with agency policies and procedures to ensure confidentiality of individual’s information both written and spoken.
• Maintain a positive and productive environment for individuals, respecting individual’s rights, and reporting suspected abuse, neglect, and exploitation of any individual.
• Spend 90% of the time in direct work with program individuals and 10% in documentation logging.
• Develop and maintain positive and productive relationship with the individuals, families, co- workers, and community members.
• Dress appropriate (Scrub top and tennis shoes or professional clothing). No low-cut clothing, high heels, or short dresses/skirts may be worn.
• Maintaining Professionalism is expected at all times while at the YOUniversity and out in the community with the individuals. Refrain from using cell phones or other electronic devices unless work-related or an emergency. Use proper language and tone while at the YOUniversity, out in the community, and with the individuals.
• Support individuals in the program in their personal growth and development respecting cultural, ethnic, spiritual and individual differences.
• Adhere to all agency policies and procedures.
• Remember we are here to help the individuals in the program first, not socialize. Please be respectful towards the individuals, their families, as well as other staff members. It is important that we set the mood for the YOUniversity to be a calm, fun, loving environment for our individuals and their families. Help the individuals have FUN and make them HAPPY!!!

QUALIFICAITONS:

• High School Diploma or GED required.
• PCA, CNA, Nurse Tech, or equivalent certification required.
• Possess 1 year of related experience preferably working with in an adult day facility, rehabilitation, or educational setting.
• Good Interpersonal skills with adults required.
• Obtain fingerprint criminal background check clearance
• Valid Georgia Drives License and 7 Year MVR with 3 or fewer points
• Reliable transportation and willingness to transport clients
