Location : Phoenix, AZ

Setting : Long-Term Acute Care (LTAC)

HumanEdge Health is seeking a skilled Travel RN Case Manager for a day shift LTAC assignment in Phoenix, Arizona. This 13-week contract offers a unique opportunity to support patients with complex needs, playing a vital role in discharge planning, care coordination, and utilization review.

Your Role Will Include :

Performing utilization reviews, resource management, and discharge planning

Collaborating with the interdisciplinary care team, patients, and families to coordinate safe and effective care transitions

Documenting treatment plans and progress notes in compliance with payor and policy standards

Assisting with referrals, external resources, and financial risk management

Leading family / team conferences and ensuring satisfaction across patients, families, and payors

Helping identify and eliminate unnecessary treatments, tests, or appointments

Managing outliers and maintaining compliance with national and local contract requirements

Qualifications :

Current RN license in Arizona (or compact license)

2+ years of acute care or case management experience preferred

BLS certification required

Familiarity with MedHost, UR tools, or commercial payor systems a plus

Strong clinical judgment, organizational skills, and team collaboration
