Care Coordinator (bilingual Eng-Span)

POSITION SUMMARY

CLUES' Certified Community Behavioral Health Clinic (CCBHC), Abriendo Caminos , provides immediate access to supportive, high-quality, culturally responsive and integrated Behavioral Health services and pathways to wellness for the Latinx Community, regardless of insurance status or ability to pay. The CCBHC model provides CLUES with a model for sustainable Behavioral Health service provision, which translates to increased access to consistent care for wellness in the Latinx community.

CLUES' Care Coordinator collaborates with Behavioral Health leadership and staff to provide integrated care within the CCBHC model. The position is responsible for promoting behavioral health services both internally within CLUES and externally within the community, while also collaborating with staff in areas including but not limited to quality, outcome-based design, service relevance, delivery, effectiveness, and evaluation, all in alignment with CLUES mission.

Position Type : Regular

Schedule : Full-Time. Generally, Monday - Friday Office hours.

Estimated Salary / Pay : $21.25 to $25 (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Responsible for working in partnership with their clients to develop and implement an integrated plan of care that will assist each client in addressing medical, behavioral, social, educational, and other service needs.
• Responsible for helping clients transition between levels of care in behavioral health.
• Coordinate services and collaborate with multidisciplinary care teams to assure high quality care.
• Responsible for helping clients to complete MNsure applications.
• Provides educational resources to clients and shared family members about the health care system and related support systems.
• Maintain up-to-date client documentation, including case notes and agency required reporting.
• Collaborate with the Behavioral Health leadership and staff to provide integrated care within the CCBHC model.
• Promote the mission, operating values, and agency services of CLUES within the community.
• Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards.
• Participate in individual and group supervision as required within the team and the department.
• Carry out all other duties as assigned by supervisor.

Education, Experience & Key Abilities

Language Skills

Effective communication in English and Spanish, both verbally and in writing, required.

Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals.

Ability to effectively present ideas and information in an organized and effective manner, including writing reports.

Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties.
