Physical Therapist PT - Home Health - Full-time

Key Responsibilities:

• Conduct thorough assessments of patients’ physical condition and mobility needs.
• Develop individualized treatment plans aimed at improving patient mobility and quality of life.
• Administer physical therapy treatments and techniques in accordance with best practice standards.
• Educate patients and their families on exercises and strategies to continue progress at home.
• Collaborate with other healthcare professionals to ensure a comprehensive approach to care.
• Maintain accurate patient records and documentation.

Qualifications:

• Valid state license to practice as a Physical Therapist.
• Strong understanding of physical therapy principles, techniques, and equipment.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Empathy and compassion for patients and their families.
• Experience in home health is a plus, but not required.
