This role focuses on providing education, training, and support to patients who choose to dialyze at home. The Home Therapy RN serves as a care coordinator, educator, and advocate, ensuring patients and their caregivers receive the necessary training for peritoneal dialysis (PD) or home hemodialysis (HHD).

• Coordinate and deliver patient training programs for home dialysis (PD / HHD) in compliance with CMS regulations.
• Conduct home visits to assess the environment's suitability for home dialysis and ensure patient safety.
• Act as a patient advocate and educator, ensuring informed consent for treatment decisions.
• Collaborate with the interdisciplinary care team, including nephrologists, social workers, and dietitians, to develop individualized treatment plans.
• Provide ongoing support and monitoring, ensuring patients are confident and competent in managing their home dialysis therapy.
• Identify opportunities for professional development, including continuing education and research application.
• Support quality assessment and improvement initiatives to enhance patient care and program effectiveness.
• Ensure compliance with all regulatory and safety requirements for home dialysis therapies.
