Job Description

Seeking an experienced LVN / LPN Case Manager for acute care Case Management & Utilization Review. Responsible for coordinating safe patient transitions, ensuring medical necessity, reviewing level of care, discharge planning, and payer authorization processes. Works closely with interdisciplinary teams and post-acute services.
