We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team as a ONE ON ONE Shift Care (PRIVATE DUTY) Home Health LVN. In this role, you will provide exceptional care and support ONE chronically ill, but stable patient in the comfort of their own homes. Your primary responsibility will be to deliver high-quality nursing services during assigned shifts, ensuring the well-being and safety of our patients. Spanish speaking is a huge plus.

​Responsibilities:

• Perform comprehensive assessments of patients' health status and develop individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor vital signs and observe any changes in patients' condition
• Assist patients with personal care activities, including bathing, grooming, and dressing
• Educate patients and their families on managing illnesses, medications, and home care procedures
• Collaborate with friendly Case Managers to ensure coordinated and effective patient care
• Maintain accurate and up-to-date electronic medical records
• Adhere to all applicable laws, regulations, and ethical standards

​Qualifications:

• Graduate of an accredited school of Nursing
• Possess a valid, current, unrestricted LVN license able to practice in CALIFORNIA - New grads welcome!
• Strong knowledge of medical terminology, tools, equipment and nursing principles
• Prior experience with G-tube, Trach, Home Vent a plus
• Ability to work independently, collaboratively and make sound decisions
• Compassionate and caring demeanor
• Excellent written and verbal communication skills
• Strong organizational and time management skills
• Reliable transportation and willingness to travel to ONE patient's home
