Description for Home Health Occupational Therapist

Acute Home Healthcare is looking for Home Health Occupational Therapist (OT) throughout Orange, Osceola, and Seminole counties to join our team!

Qualifications of Home Health Occupational Therapist (OT):

  • Currently licensed in the state(s) in which practicing

  • At least 2 year or more of experience in home care

  • Level 2 Background screening

Responsibilities and Duties of Home Health Occupational Therapist (OT):

· Improves or minimizes residual physical disabilities of the patient.

· Returns the individual to optimum and productive living within the patient's capabilities.

· Periodically participates with all other health care personnel in patient care planning.

· Provides prescribed occupational therapy.

· Takes initial history and makes initial evaluation.

· Consults with physicians regarding change in treatment.

· Writes reports to physicians regarding patient's progress.

· Instructs patients/family members in home programs and fine motor movement exercises.

· Assists the physician in evaluating level of function.

· Helps develop the plan of care and revises as necessary.

· Prepares clinical and progress notes.

· Advises and consults with the family and other Agency personnel.

· Participates in Inservice programs.
