BRIEF SUMMARY OF THE JOB:
The Social Worker is responsible for providing case management, discharge planning, and counseling services to residents and families of the Nursing Home. In addition, the Social Worker maintains contact with community agencies and individuals who can contribute to a comprehensive plan of care when appropriate, and within the context of maintain confidentiality.
DUTIES AND RESPONSIBILITIES
PHYSICAL REQUIREMENTS Must be able to move intermittently throughout the day.
QUALIFICATIONS:
