Licensed Practical Nurse Home Health

Employment Type: Full-time / Part-time
Schedule: Weekdays & Weekends required

Responsibilities:

• Follow comprehensive patient assessment E1 and be able to review and interpret individualized care plans.
• Provide skilled nursing services (medication management, wound care, IV therapy).
• Educate patients and caregivers on disease management, medications, and safety.
• Document visits accurately and in compliance with state and agency policies.
• Communicate with physicians and interdisciplinary team to update care plans.
• Participate in weekend/holiday coverage.

Requirements:

• Active Florida LPN license.
• Current CPR certification, TB test, valid drivers license.
• At least 1 year of clinical experience (inpatient and home health required).
• Valid drivers license and reliable transportation.
• Must carry Liability insurance
• Clear background check and eligibility to work in the U.S.
• Ability to work flexible hours including weekdays, weekends, and some holidays.
• Strong communication, organizational, and problem-solving skills.

Preferred Qualifications

• Previous experience in home health or long-term care.
• Woundcare and Woundvac experience is a plus
• Iv antibiotics administration experience is required
• Must be able to perform one on one with patient
• Must be able to do blood draws
• Familiarity with EMR systems.
• Bilingual (English/Spanish or English/Creole) a plus.
