Social Services LCSW (Licensed Clinical Social Worker)

LCSW position at Heart Transplant Program in Portland 13 week contract. Clinic is primarily looking for outpatient support for advanced heart failure treatment evaluations including heart transplant and occasional mechanical heart. There may be some inpatient evaluation requirements as well. Transplant clinics supports 250 total transplant patients and 40 VAD patients with an average of 15-20 transplants annually. Ideal candidate would have some experience with transplant patients and/or heart patients. Candidate will be comfortable interacting with an inter-disciplinary team.
