I founded In-Home Caregivers of Illinois as a Nurse Practitioner who saw families overwhelmed, confused, and alone in caring for aging loved ones. Our mission is to provide a nurse-led, compassionate, clinically strong aging-in-place experience for families who want their loved ones to remain safe, well, and dignified at home.
Today, demand for our services has grown—both in services and needs
We are expanding our clinical team, and I am seeking an LPN who will work directly with me, supporting patient care, skilled nursing services, caregiver development, and our wellness programs.
This is not a large nursing department.
This is a role where you help build the foundation of our nurse-led clinical home care model
Skilled Nursing Services
Perform skilled nursing visits (medication setup, vitals, wound care within LPN scope, chronic disease support, safety checks)
Support our Medication Management & Care Management Programs
Teach families how to safely understand and manage medications
Caregiver Coaching & Training
Provide caregiver coaching to improve care quality
Conduct caregiver training sessions and skill validation
Complete supervisory visits and reinforce care plans
Perform on-site skill assessments
Clinical Assessments & Oversight
Conduct intake assessments for new clients
Complete in-home supervisory visits
Monitor high-risk clients and communicate early changes
Document clinical findings promptly and accurately
Assist with safety evaluations and post-hospital transitions
Direct Partnership With the Nurse Practitioner
Work closely with me (NP) as your supervising clinical lead
Help coordinate patient care and wellness programs
Participate in developing clinical workflows and systems
Support expansion of our nurse-led model in new locations
A licensed LPN in Illinois and enjoys a combination of direct care, administrative and supervisory roles
Compassionate, confident, and committed to high-quality care
Strong communicator with families and caregivers
Able to manage clinical tasks, documentation, and follow-through

Full-time preferred: Monday–Friday, 8:30 AM–5:00 PM (30-min unpaid break) with administrative on-call one week a month
Part-time option for the right candidate (Must be Monday to Friday)
