Client Care Coordinator

A reputable organization in Queens, NY is seeking a Client Care Coordinator to join their team at a renowned family shelter. This position offers an opportunity to make a meaningful impact by providing top-tier case management services to families in need. About the Opportunity: Schedule: Monday to Friday Hours: 9am to 5pm (35 hours/week) Setting: Family Shelter Responsibilities: Managing a caseload of approximately 50 families Completing biopsychosocial evaluations within 30 days of client entry Conducting weekly individual sessions with clients Leading "Ready Set Goal" group sessions for young parents Creating safety plans for domestic violence situations Qualifications: Master's in Social Work required LMSW within one year of hire TB test, First Aid/CPR, and overdose prevention certifications Solid assessment and documentation skills Great interpersonal skills Excellent communication skills Organizational skills Desired Skills: NYS Licensed Master Social Worker (LMSW) Bilingual (English and Spanish) Experience with Biopsychosocial Assessments
