Certified Nursing Assistant (CNA) - Comprehensive Job Description

Role Summary

The Certified Nursing Assistant (CNA), also referred to as a Hospice Aide, is a paraprofessional member of the interdisciplinary group. The CNA works under the direct supervision of a Registered Nurse Case Manager to provide personal care and essential supportive services to patients in their place of residence (private homes, assisted living facilities, etc.). The primary goal is to promote the patient's comfort, hygiene, and overall quality of life.

Key Responsibilities

1. Patient Personal Care and Comfort :

  • Assist patients with Activities of Daily Living (ADLs) including bathing (bed, shower, or tub), dressing, grooming, and oral / mouth care.
  • Provide respectful and discreet assistance with toileting, incontinence care, and the use of bedpans, urinals, or commodes.
  • Assist with mobility, transfers (bed to chair, etc.), and ambulation.
  • Perform turning and repositioning of bed-bound patients to prevent pressure sores and ensure comfort.
  • Provide non-medical comfort measures, such as skin care, back rubs, and repositioning.

2. Observation, Documentation, and Reporting :

  • Accurately take and record vital signs (temperature, pulse, respiration, and blood pressure) as instructed on the care plan.
  • Serve as the "eyes and ears" of the hospice team by observing, documenting, and promptly reporting any changes in the patients physical condition, mental status, behavior, or comfort level to the supervising Registered Nurse Case Manager.
  • Maintain clear, concise, and timely electronic or written documentation of all care provided and observations made in accordance with organizational policies.

    • 3. Supportive and Environmental Tasks :

  • Assist patients with nutritional support, including preparing and serving light meals and ensuring adequate hydration, as assigned.
  • Perform light housekeeping duties related to the patients immediate environment, such as changing bed linens, tidying the patient's room, and light laundry.
  • Ensure a safe, clean, and comfortable environment for the patient, including the safe and proper use of durable medical equipment (DME).
  • Run occasional, patient-related errands (e.g., picking up equipment or approved supplies).

    • 4. Emotional and Family Support :

  • Provide compassionate companionship, comfort, and emotional support to the patient and family.
  • Use active listening skills and a gentle, empathetic presence to support patients and caregivers during a sensitive time.
  • Provide respite support to family caregivers as part of the scheduled visit.
  • Adhere to all patient rights and maintain strict confidentiality.

    • Minimum Qualifications & Requirements

  • Certification : Current, unencumbered Certified Nursing Assistant (CNA) certification or registration in good standing with the state nurse aide registry.
  • Education : High school diploma or equivalent.
  • Experience : Minimum of one (1) year of experience as a CNA, preferably in a hospice, home health, long-term care, or hospital setting.
  • Licensure : Valid drivers license and reliable transportation with current automobile insurance.
  • Skills : Demonstrated ability to work independently, excellent interpersonal and communication skills (verbal and written), and a high level of patience, compassion, and emotional resilience.
  • Physical : Ability to perform the physical requirements of the job, including lifting up to 50 lbs., bending, stooping, and prolonged periods of standing / walking.
  • Other : Current CPR certification. Must pass all background checks and health screenings as required by state and federal regulations.
