Responsibilities

What You Will Do:

• Organize, develop, manage, and direct the home health functions of your community health hub
• Manage a team of clinicians to ensure our patients receive exceptional care in their homes
• Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's needs
• Conduct patient visits (as needed)
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

Qualifications:

• Active RN state license
• Two or more years of nursing experience (supervisory experience preferred)
• Two or more years of home health nursing experience required
• Valid drivers' license
