Pay Range : $33 / hour - $59 / hour.Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities :

• Direct Patient Care : Perform comprehensive patient assessments, develop nursing diagnoses, and implement / evaluate individualized care plans using evidence-based practice and analytical thinking.
• Patient Education & Advocacy : Provide relationship-based, patient-centered care and education tailored to population-specific needs (e.g., chronic disease management). Partner with patients and families to keep them informed and make patient goals the focus of care.
• Care Coordination & Teamwork : Coordinate the delivery and documentation of safe, quality patient care. Foster a respectful, inclusive clinical environment through effective interdisciplinary communication and collaboration. Participate in performance improvement initiatives.
• Clinical Operations : Prioritize and coordinate optimum daily patient flow and experience. Deliver safe and effective care according to clinic policies and procedures.
• Professional Development : Complete all required competencies and education. May serve as a preceptor or mentor, modeling professional nursing practice and contributing to a healthy work environment.
• Quality & Safety : Uphold all organizational policies, procedures, and clinical competencies. Participate in data collection and evidence-based practice to support quality outcomes and patient safety goals.
