Overview of the Role:

Exciting Opportunities at Alignment Health! As an Advanced Practice Clinician (APC) you will be part of a transformative program designed to provide personalized care and support to our patients, including those with complex and chronic needs, all in the comfort of their own homes. We have 2 clinical programs that we are hiring for.

The APC will ensure the delivery of high-quality clinical and home-based patient care, while promoting optimal health outcomes and providing cost-effective solutions that adhere to Alignment Health s policies, as well as state and federal regulations.

You ll play a vital role in assessing, developing, and coordinating care options tailored to the needs of our members, who are typically 65 and older. With home and virtual visits ranging from 45 to 60 minutes, you ll have the opportunity to provide focused, compassionate care, whether it s an initial visit, an annual check-up, or when additional support is needed to address medical and social challenges.

Approximate Salary Range: $150,000 to $160,000 Yearly
