Job Title: RN Home Health Address: Moline, IL Job Description Administer skilled nursing care for clients of all ages in their place of residence. Coordinate care with the interdisciplinary team, patient/family, and referring agency. Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs. Implement, develop, document, and evaluate the Plan of Care (POC). Educate patients and/or their families regarding care, diet, and home safety. Supervise and provide clinical directions to the healthcare team to ensure quality service. Perform on-call responsibilities and provide on-call services as assigned. Critical Information Schedule: Mon-Fri, no holidays, no weekends, no on-call. Coverage area includes Quad City area on both IA and IL (Rock Island IL, Moline IL, Davenport IA, Bettendorf IA). 36 hours guaranteed per week. Education/Licenses Needed Current state license as a Registered Nurse. Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program. CHPN certification (preferred). Proof of current CPR certification. 1-2 years experience as a Registered Nurse in a clinical care setting. 2+ years of Home health experience.
