[job_card.job_description]Home health setting serving a largely geriatric patient population Covers a territory up to a 60-mile radius, including Valencia County and the East mountains Average caseload of 5-7 patients per day Day shift, Monday through Friday, 8 : 00 am – 5 : 00 pm On-call required every 3rd to 4th weekend Uses HCHB EMR system; experience required iPad provided for work documentation Mileage reimbursed at the IRS standard rate Main responsibilities include evaluation, treatment, and coordination of care for geriatric home health patients First-time travelers are accepted for this position Locals are accepted at travel rate Scrubs required; any color is acceptable but jeans are not permitted Orientation and training provided as needed Active PT license required if applicable Minimum 1 year of recent home health PT experience required Current BLS certification required Proficiency with Oasis assessments and HCHB EMR required Must be flexible with assigned territory and travel Timely documentation and clear communication skills required High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
