Job Description\n

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of recent experience, BLS certification, and proficiency in Oasis and HCHB EMR.

\n \n Responsibilities\n

  • Home health setting serving a largely geriatric patient population
  • Covers a territory up to a 60-mile radius, including Valencia County and the East mountains
  • Average caseload of 5-7 patients per day
  • Day shift, Monday through Friday, 8 : 00 am – 5 : 00 pm
  • On-call required every 3rd to 4th weekend
  • Uses HCHB EMR system; experience required
  • iPad provided for work documentation
  • Mileage reimbursed at the IRS standard rate
  • Main responsibilities include evaluation, treatment, and coordination of care for geriatric home health patients
  • First-time travelers are accepted for this position
  • Locals are accepted at travel rate
  • Scrubs required; any color is acceptable but jeans are not permitted
  • Orientation and training provided as needed

  • Active PT license required if applicable
  • Minimum 1 year of recent home health PT experience required
  • Current BLS certification required
  • Proficiency with Oasis assessments and HCHB EMR required
  • Must be flexible with assigned territory and travel
  • Timely documentation and clear communication skills required
