What You'll Be Doing
The Nurse Practitioner (NP) will be responsible for providing comprehensive care to patients in a single skilled nursing facility. This is a Monday–Friday role with no night or weekend call , designed for clinicians who want to focus on delivering high-quality, efficient, and patient-centered care. You will collaborate with the facility staff, physicians, and other members of the healthcare team to ensure timely, effective, and coordinated care.
Key Responsibilities
Provide direct medical care to patients in the assigned facility, including assessment, diagnosis, treatment, and follow-up.
Manage acute and chronic conditions, focusing on high-quality, efficient care delivery.
Coordinate with facility staff, specialists, and families to develop and update individualized care plans.
Support transitions of care, including post-hospital evaluations, medication reconciliation, and patient / family education.
Maintain thorough, timely, and compliant documentation of patient encounters and care plans.
Participate in quality initiatives and process improvements that enhance patient outcomes and efficiency.
Collaborate with the interdisciplinary team to ensure continuity of care and adherence to evidence-based protocols.
Skills & Abilities
Strong clinical knowledge of adult and geriatric medicine, with expertise in chronic disease management.
Excellent communication and collaboration skills with patients, families, and facility staff.
Ability to manage a patient panel efficiently while maintaining a high standard of care.
Strong organizational and documentation skills.
Commitment to patient-centered, evidence-based practice.
Qualifications
Current Nurse Practitioner license in the state of Georgia.
Certification in Internal Medicine, Geriatrics, or Family Practice required.
Must have an unrestricted DEA license
Minimum 1–2 years of NP experience preferred; experience in post-acute, SNF, or geriatrics strongly preferred.
Ability to work independently in a facility setting with physician support.
Familiarity with EHR systems and clinical documentation best practices.
