Make an Impact as a Care Coordinator

The NewPath Child & Family Solutions Care Coordinator is responsible for coordinating the implementation of the Plan of Care, including a Transition and Crisis Plan, through service delivery with providers and community resources. The Care Coordinator cultivates flexible, family-focused, community-based responsive services based on the High Fidelity Wrap Around model of care coordination.

This is is a full time position located at our NewPath Central Campus, 4721 Reading Rd. Cincinnati, OH 45237
