Position Type : Full time.

Start Date : July start date

Schedule : Shifts are 4x10's 8am-5pm. You have flexibility to choose which four days between Monday-Friday

Pay Rate : Negotiable by phone.

About the Role

We are seeking a dedicated and experienced hospice / geriatric NP able to travel in the Maumee / Toledo area providing care to patients at homes, SNFs, and LTC facilities.

Qualifications

• Board Certified (BC) in FNP, AGNP, or ACAGNP
• OH state license
• Current or recent experience in hospice or palliative care (within last 2 years)
• Strong communication and patient care skills
