RN- Registered Nurse (Home Health)

Job Summary

Key Responsibilities

• Conduct comprehensive in-home patient assessments and develop personalized care plans.
• Administer medications, injections, IV therapies, and wound care.
• Monitor patient health status and promptly communicate changes to the supervising physician or care team.
• Educate patients and caregivers on medication management, chronic condition care, and overall health practices.
• Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards.
• Collaborate with multidisciplinary teams including therapists, home health aides, and physicians.
• Promote patient safety and ensure a clean and supportive home environment.

Qualifications

• Active and unencumbered Registered Nurse license in Texas.
• Minimum of 1 year nursing experience, preferably in home healthcare or community nursing.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Strong clinical judgment, independence, and time management skills.
• Comfortable working in various home environments.

Preferred Qualifications

• Familiarity with OASIS documentation and EMR systems.
• Experience managing chronic conditions such as diabetes, CHF, or COPD.
• Multilingual skills are a plus.
