Key Responsibilities

Evaluates team performance and provides coaching to improve continuity of care

Required Qualifications

Bachelor's degree required

4+ years of related experience preferred

Experience in care coordination and team supervision

Knowledge of regulatory guidelines and standards

Ability to oversee and train care coordination team members
