Department Details

The Skilled Rehab and Home Health team is looking for a passionate Physical Therapist to join our team. In this full-time role, working 35-40 hours per week, you will support our Skilled Nursing Facilities and home health clients. We treat a variety of conditions, including orthopedic issues, weakness, balance problems, and pain management. You will work alongside 1 full-time PT and 2 full-time PTAs. New graduates are encouraged to apply! We value teamwork and are searching for someone who can enhance our collaborative environment. You will receive paid travel and mileage between home health patients.

Job Summary

As a Physical Therapist, you will :

• Plan, organize, and deliver innovative physical therapy programs to help clients regain mobility, alleviate pain, and prevent further issues.
• Conduct comprehensive examinations, evaluations, and interventions for clients facing various injuries or diseases.
• Document client information accurately, establish therapeutic plans, and modify them as necessary.
• Educate clients and families on treatment plans and at-home exercises to ensure continued progress.
• Utilize current research to inform your practice and treatment methods.

You will interact with individuals of all ages and supervise assistants, aides, and students in their patient care roles. Flexibility in work schedules is important, and you will collaborate with providers, case managers, and other stakeholders. Your expertise will also contribute to ongoing education within the department.

Based on facility needs, you may focus on post-acute, long-term care, and home healthcare settings.
