Registered Nurse (RN) or Licensed Practical Nurse (LPN)

Part-Time and PRN | 7am–7pm | 3 Days per Week + Every Other Weekend

Plum Creek Chicago s seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our care team. This role plays a vital part in delivering high-quality, person-centered care aligned with each resident’s individualized assessment and care plan — under the guidance of our Wellness Director.

Key Responsibilities

Provide daily nursing care and clinical services to residents.

Perform health assessments and monitor vital signs.

Partner with CNAs and care staff to identify resident needs and changes in condition.

Administer medications and maintain accurate documentation.

Offer clinical leadership and support to the care team.

Foster a positive, resident-focused environment.

Complete additional tasks as assigned by the Executive Director.

Qualifications

Current Illinois RN or LPN license in good standing.

Recent experience in a Memory Care or Supportive Living environment preferred.

Strong clinical, assessment, and supervisory skills.

Proficiency in medication administration and care documentation.

Requirements

High school diploma or GED.

Completion of a state-approved nursing assistant program.

Active status on the IDPH Health Care Worker Registry.

Ability to lift up to 25 lbs and perform typical nursing and CNA support duties.

Valid CPR certification (recently expired certifications accepted; recertification offered).
