Registered Nurse

We are seeking Registered Nurses to join our Team! You will be responsible for the assessment, management, and treatment of assigned patients.

Responsibilities Include:

• · Visit clients in the privacy of their residence in designated geographic territories.
• · Schedule and preform monthly supervisory visits
• · Schedule and preform Quarterly Evaluations
• · Complete timely and accurate clinical notes including addressing patient's progress.
• · Evaluate the client status and the skills of workers while the staff member is present.
• · Keeps documentation of all communication with clients, family members and support staff. .
• · Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
• · Counsels and educates the patient and family in meeting healthcare and related needs.
• · Write 485/ Plan of Care for patients when reauthorizations are due for patients.
• · Other projects/duties as assigned.

Requirements:

• · Home Care experience preferred
• · Excellent verbal and written communication skills.
• · Highly motivated, organizational, leadership and relationship building skills
• · Ability to work in a very detailed, fast paced environment
• · Currently holds a RN license in PA
• · Valid state driver’s license and car insurance
