Physical and Occupational Therapist for Home Health Care

We are seeking a self-managed and encouraging Physical and Occupational Therapist to provide therapy services as prescribed by a physician in our clients’ homes. You will assist the physician in evaluating patients through the application of diagnostic and prognostic muscle, joint, and functional abilities tests while observing and recording findings in the patients’ conditions. You will also instruct other health team personnel and family members in certain phases of therapy so they may more aptly work with the patient to improve their physical condition. Job Responsibilities

As a Therapist for Tendercare Home Health Services, you will treat patients in their homes to restore their function, alleviate pain, prevent permanent disability and generally improve their quality of life and overall independence. You will see orthopedic and geriatric patients, individuals with neurological disorders and trauma victims on a daily basis. Pediatric clients may also be available.

Additional responsibilities for the Therapist include :

• Writing patients’ care plans and evaluations
• Evaluating clients’ homes for any safety barriers and educating in fall prevention
• Assisting with transfers, bed mobility, gait, and functional mobility
• Assessing clients’ mental status and physical abilities
• Documenting all treatment that was provided to the patient and revising and updating goals
• Educating patient and caregivers on the patients’ condition and instructing in-home exercise program to carry over progress
• Assessing a patients’ ADL and IADL needs and limitations
• Evaluating for equipment needs that may need to be installed in the home for safety purposes
• Assist in acquiring any adaptive, ADL, or equipment needs and train in proper and safe use
• Developing clients’ balance, strength, and overall range of motion
• Completing all patient functional assessments, re-certification, and discharge paperwork
