Licensed Clinical Social Worker (on-site)

Northwest Specialty Hospital, an extension of Northern Nevada Medical Center, brings expanded services to a new building located at 6225 Sharlands Ave. in Northwest Reno. The facility will offer acute rehabilitation and adult behavioral health services.

Our acute rehabilitation services include occupational, speech, and physical therapy for adults recovering from stroke, injury, or another disabling condition. We offer advanced treatment for a wide range of conditions including stroke, brain injury, amputation, neurological disorders, major multiple traumas and spinal cord and back injuries. All patients will experience private rooms, have access to a newly renovated therapy gym, and personalized care from our qualified staff.

The adult behavioral health services offered is an expansion to our current 50 and older program at NNMC. The new program will offer behavioral health services to all adults 18 years and older. The department is managed through a multidisciplinary team including physicians, nurses, CNAs, social workers, and therapists. Patients will experience an individualized care plan, inclusive of individual and family therapy, activities, medication management, and a 24-hour secure, compassionate environment.

This Licensed Clinical Social Worker opportunity is full time at 40 hours per week, offers full benefits and a convenient day shift schedule. We are looking for a dynamic individual that will be responsible for the assessment, recommendation, the appropriate and timely discharge and transfer arrangements for patients in the ED that have been placed on a legal hold, have an underlying psych condition, and / or present under the influence of a substance (alcohol or drug).

Job Duties / Responsibilities :

• Completes evaluation of patients on legal hold to assess safety needs and appropriate level of care.
• Works with multidisciplinary team to make recommendations on continuing or discontinuing legal hold.
• Discharge planning, gathering and analyzing data pertaining to patients needs and resources.
• Assists patients and their families in the application for financial programs.
