Our services include skilled nursing, therapy services, personal care, companion support, medication management, Alzheimers / dementia care, and developmental disability services.

We are growing and seeking a dedicated Physical Therapist Assistant (PTA) to join our team and support clients in achieving their highest level of independence and function.

Position Summary

The Home Health PTA will work under the direction and supervision of a licensed Physical Therapist to provide treatment, restore mobility, improve strength, and enhance the overall quality of life for clients in their homes. This role requires strong communication skills, clinical competency, and the ability to work independently in a community-based setting.

Key Responsibilities

  • Provide therapeutic exercises and physical therapy interventions as delegated by the PT.
  • Assist clients with mobility training, balance improvement, endurance, and functional task performance.
  • Observe, document, and report changes in client condition to the supervising PT and clinical team.
  • Complete accurate, timely visit documentation in accordance with state and agency standards.
  • Educate clients and caregivers on home exercise programs, safety, and physical therapy goals.
  • Collaborate with interdisciplinary team members to support the clients overall care plan.
  • Travel to client homes within service areas (mileage reimbursement available).

Qualifications

  • Valid Ohio Physical Therapist Assistant (PTA) License required.
  • Current CPR / First Aid certification.
  • Prior home health experience preferred but not required.
  • Reliable transportation, valid drivers license, and active auto insurance.
  • Excellent communication, professionalism, and time-management skills.
