Patient Escort & Companion Job Summary:

We are seeking compassionate and caring individuals to join our team as a Patient Escort\Companion at Pinnacle Multicare Nursing and Rehabilitation Center in The Bronx, New York. As a key member of our care team, you will play a vital role in ensuring the comfort, safety, and well-being of our patients. Patient Escort & Companion Responsibilities:

• Transport patients to and from medical appointments.

• Transport patient’s clothes and property from one unit to another.

• Maintain records of activates including the time patient is picked up, time of arrival at a specified area and time of patient’s return to their designated area.

• Unit hall monitoring

• Escort hospital and ambulatory patients traveling by wheelchair, stretcher, or hydraulic stretcher safely and efficiently.

• Assist in transferring patients between beds and transportation equipment.

• Assist with transport for patients being discharged.

• Escort patients to and from various areas of the facility, including therapy sessions, meals, and activities

• Provide companionship and conversation to patients, engaging in activities such as reading, playing games, or watching movies

• Assist patients with personal care tasks, such as bathing, dressing, and grooming

• Help patients with mobility and transfers, ensuring their safety and comfort

• Monitor and report any changes in patients' conditions or needs to the nursing staff

• Maintain a clean and organized environment, adhering to infection control protocols

• Collaborate with the interdisciplinary care team to develop and implement individualized care plans

Patient Escort & Companion Requirements:

• Ability to lift, transfer, and reposition patients safely and comfortably

• Ability to push wheelchairs, stretchers, bed stretchers and gurneys.

• Excellent communication and interpersonal skills

• Compassionate and patient-centered approach to care

• Ability to work varying shifts, including evenings and weekends
