Job Description
The Case Manager provides field-based support and coordination for adults living with moderate to severe mental health conditions. This role focuses on intensive case management, psychosocial rehabilitation, and helping clients achieve stability and independence through individualized support and resource connection.
Responsibilities/ Job Duties:
- Provide intensive case management and psychosocial rehabilitation services to adult clients.
- Collaborate with multidisciplinary teams, including psychiatrists, clinics, and community service providers, to ensure coordinated care.
- Develop, monitor, and reinforce client treatment plans.
- Conduct regular field visits and maintain consistent client contact throughout the week.
- Transport clients to and from appointments, including medical and social services.
- Link clients to community programs, benefits, housing, and other support resources.
- Participate in team meetings and contribute to care coordination and service planning.
- Respond to client crises as needed, maintaining a calm and supportive approach.
- Maintain accurate and timely documentation of services provided.
- Support team members and provide coverage as needed in a collaborative, "whatever it takes" environment.
Minimum Education and Experience Qualification Requirements:
- Prior experience in case management or community-based behavioral health services.
- Knowledge of local community resources and social service systems (Ventura County knowledge preferred).
- Experience working as part of a multidisciplinary team.
- Strong communication, organization, and crisis intervention skills.
- Valid driver's license and proof of auto insurance required.
