Your Role as a Home Health Speech-Language Pathologist :

  • Deliver home-based speech therapy to patients struggling with communication, swallowing, or cognitive difficulties resulting from illness or injury.
  • Collaborate within a home health team, including RN, LPN, CNA, HHA, PT, and OT, to follow the patient’s care plan and achieve established goals.
  • Assess and evaluate each patient's communication, swallowing, and cognitive abilities.
  • Document patient progress, conduct assessments, and ensure goals are met effectively.
  • Guide patients through mouth, breathing, and swallowing exercises, fluency, articulation, and reading exercises.
  • Educate patients and their families on care plans, therapy goals, safety practices, and self-care techniques.

    • Essential Qualifications :

  • Master's or Doctoral degree in Speech-Language Pathology and an active SLP license in Michigan.
  • At least 2 years of experience in speech therapy, preferably in home healthcare settings.
  • Current CPR certification.
  • Familiarity with state and federal home health regulations.
  • Exemplary clinical judgment, critical thinking abilities, strong communication skills, and a genuine sense of empathy and compassion.
