[job_card.job_description]Deliver personalized hospice care that honors each patients unique needsdocument every step to ensure accuracy and continuity. Lead with empathy during home visits , assessing patient and family needs and crafting care strategies that prioritize comfort, dignity, and peace. Empower families and caregivers through clear education on hospice services, symptom management, and available support resources. Guide and mentor your team , training Hospice Aides and LPNs to deliver safe, effective, and compassionate care at home. Collaborate seamlessly with physicians, interdisciplinary teams, and facility staff to create smooth transitions and coordinated care plans. Provide hands-onexpertise in pain control, medication administration, and symptom reliefbringing comfort when it matters most. Be a trusted resource after hours ,participatingin on-call rotations to ensure patients and families receivetimelysupport on weekends and holidays.
